Medical myths: All about weight loss

 As 2021 begins, many of us might be planning to lose some weight. However, when it comes to dieting and weight loss, confusion abounds. Here, we dispel 11 widely held myths.

During the holiday season, many people take on more calories than they work off. Consequently, adding a little extra weight at this time of year is not unusual.

Classically, January is a time of change, and many people go on health kicks that include weight loss. With that in mind, we address some common misconceptions.

The common adage that breakfast is the most important meal of the day may or may not be true, but it seems unlikely that skipping the morning meal aids weight loss.

The rationale behind this strategy is that missing one meal a day leads to a lower overall intake of calories. However, the story is not so straightforward.

One studyTrusted Source, published in 2010, analyzed food intake information from 2,184 people aged 9–15 years. Twenty years later, the researchers asked for the same information again.

They compared data from people who had skipped breakfast during childhood and adulthood with data from those who had never skipped breakfast or had done so only in adulthood.

Compared with the other groups, the participants who skipped breakfast during both childhood and adulthood tended to have larger waist circumferences, higher fasting insulin levels, and higher total cholesterol levels.

Sometimes, people who skip breakfast eat more during the rest of the day to counteract the deficit. But one 2013 studyTrusted Source found that missing breakfast does not lead to eating more at lunch. The authors conclude that “Skipping breakfast may be an effective means to reduce daily energy intake in some adults.”

However, these researchers only monitored the participants’ food intake at lunch, not dinner. And the study only included 24 participants, so we should be wary of drawing solid conclusions from the findings.

A much larger 2007 studyTrusted Source, which involved more than 25,000 adolescents, looked for links between skipping breakfast and having overweight. The researchers also assessed the roles of alcohol intake and levels of inactivity.

The scientists found that skipping breakfast had a stronger association with overweight than either alcohol consumption or levels of inactivity.

A 2020 systematic review and meta-analysis that appears in the journal Obesity Research & Clinical Practice concurs. After analyzing the results of 45 previous studies, the authors concluded:

“Skipping breakfast is associated with overweight/obesity, and skipping breakfast increases the risk of overweight/obesity.”

Certain foods are referred to as “fat-burning.” This sounds too good to be true, and it probably is.

Some people claim that foods such pineapple, ginger, onions, avocados, asparagus, celery, chilies, broccoli, green tea, and garlic speed up the body’s metabolism, thereby helping the body burn fat.

There is little scientific evidenceTrusted Source, however, that these foods can help reduce weight.

Proponents of certain supplements claim that they, too, help the body burn fat. In reality, these are generally ineffectiveTrusted SourcedangerousTrusted Source, or both.

The Food and Drug Administration (FDA)Trusted Source report that they have “found hundreds of products that are marketed as dietary supplements but actually contain hidden active ingredients […] contained in prescription drugs, unsafe ingredients in drugs that have been removed from the market, or compounds that have not been adequately studied in humans.”

Jason Humbert, a senior regulatory manager at the FDA, adds:

Of course, reduced fat or low fat foods are likely to contain less fat. However, these products sometimes compensate with added sugar or salt. Checking labeling is key.

Also, it is worth noting that “reduced fat” does not necessarily mean “low fat,” but simply that the fat content of a product is lower than the full-fat version.

“We’ve also found weight loss products marketed as supplements that contain dangerous concoctions of hidden ingredients, including active ingredients contained in approved seizure medications, blood pressure medications, and antidepressants.”

 5. Thou shalt not snack

People may believe that snacking is a cardinal sin of dieting. In some cases, though, snacking can help people manage their caloric intake more effectively. It is not so much snacking itself, but what one snacks upon that that counts.

Snacking may be more complex than it first appears. While some snack on celery and others on cookies, some people might snack because they are hungry and others because they are bored.

Having a piece of fruit or some low fat yogurt between meals, for instance, might reduce food cravings, keeping a person from overeating at mealtimes or resorting to more energy-dense snacks.

Some researchTrusted Source has shown that people with obesity are more likely to snack than people without the condition. In this case, switching from unhealthful to healthful snacks may aid in weight loss, if this is a goal.

In general, snacking is multifaceted — it may support or hinder weight loss efforts, and in some cases it may have little effect.

Older researchTrusted Source, from 2007, investigated the role of snacking in weight loss. In the yearlong study, the researchers asked one group of participants to have three meals a day, without snacking. A second group was asked to have three meals and three snacks daily. Individuals in both groups consumed equivalent amounts of energy each day.

At the end of the study, the authors found no difference in weight loss between the two groups; snacking, it seems, neither helped nor hindered.

Another study, from 2011, investigated the “relationship between eating frequency and weight loss maintenance.” The researchers recruited participants who had previously had overweight or obesity but who had since maintained healthy weights.

They compared these individuals’ eating patterns with those of people with obesity and people with healthy weights who had never had overweight.

The team found that participants with healthy weights ate more snacks than those who had lost weight — and that people with overweight ate the fewest snacks.

Overall, the authors concluded that “Eating frequency, particularly in regard to a pattern of three meals and two snacks per day, may be important in weight loss maintenance.”

This follows on from the myth above. Of course, limiting sugary, high fat treats is important, but cutting them out entirely is unnecessary and could be counterproductive.

As the British Heart Foundation explain:

“Depriving yourself of all the foods you enjoy won’t work. You’ll eventually give into temptation and abandon your efforts. There’s no harm in allowing yourself a treat now and again.”

There is a rumor that minimally processed sugars, such as those in maple syrup or honey, are more healthful than white sugar. In reality, our bodies process sugar in the same way — regardless of its source. The gut reduces all sugars into monosaccharides.

Rather than looking at sugar processing, it is more important to note the amount of sugar in any food. All types of sugar provide around 4 calories per gram.

Following on from the myth above, we know that all sugar is high in calories. However, a person looking to lose weight does not need to mercilessly cut sugar from their diet.

As with all things, moderation is key. It may instead be a good idea to avoid products with added sugar.

To lower their sugar intakes, many people opt for low- or no-calorie sweeteners, such as aspartame. This may reduce the number of calories consumed, but some studies have linked artificial, or nonnutritive, sweeteners to weight gain.

systematic reviewTrusted Source and meta-analysis published in 2017 analyzed 37 existing studies involving a total of 406,910 participants to investigate the impact of sweeteners on cardiometabolic health.

According to the researchers, “Observational data suggest that routine intake of nonnutritive sweeteners may be associated with increased BMI [body mass index] and cardiometabolic risk.”

However, not all studiesTrusted Source have reached this conclusion. Researchers and health experts continue to discuss the effects of nonnutritive sweeteners on weight loss and metabolic health.

Some people are particularly keen to lose fat from certain areas, such as the thighs or abdomen. In reality, this targeting is not possible. All bodies respond differently to weight loss, and we cannot choose which bits of fat will go first.

However, if weight loss is paired with exercises to tone a particular area, it can give the impression of more region-specific weight loss.

Vast legions of diets have become fashionable only to fade into obscurity, making room for more. As the Centers for Disease Control and Prevention (CDC)Trusted Source explain in a nutshell:

“[Fad] diets limit your nutritional intake, can be unhealthy, and tend to fail in the long run.”

Overall, losing weight can be challenging. Our bodies evolved during leaner times, so they are primed to take on energy and store it. However, many of us live in a world where calories are readily available, and our bodies still store this energy as if it’s going out of fashion.

In general, reducing the caloric intake and exercising is the most reliable approach to weight loss. With that said, people with chronic diseases, including diabetes and obesity, should speak with their doctors before beginning a new weight loss regime.

It is also worth remembering that if anything seems too good to be true, it probably is — any “weight loss miracle” is unlikely to be miraculous. Most foods are not inherently unhealthful, but it is a good idea to consume high-sugar and high-fat foods sparingly.

How much protein do you need to build muscle?

 While strength training is important for building muscle, so is consuming the right amount of protein. There has been continuous research and controversy about how much protein is needed to optimize muscle growth. In this Honest Nutrition feature, we discuss current research evaluating the role of protein in muscle growth and how much a person should consume each day.

Protein is found in every cell and tissue in the body. While it has many vital roles in the body, protein is crucial for muscle growth because it helps repair and maintain muscle tissue.

The current recommended dietary allowanceTrusted Source (RDA) to prevent deficiency in minimally active adults is 0.8 grams (g) of protein per kilogram (kg) of body weight. However, newer research suggests individuals trying to build muscle need more than this.

Consuming less protein than the body needs has been linked to decreased muscle mass. In contrast, increased protein intakes above the RDA may help increase strength and lean body mass when paired with resistance exercise.

Protein is made up of amino acids that act as building blocks for cells and tissues in the body. There are 20 amino acids that combine to form proteins.

While some can be synthesized by the human body, others cannot. The nine amino acids that the body cannot make are called essential amino acids. These must be obtained through diet.

When a person eats protein, it is digested and broken down into amino acids, which are involved in many processes in the body, including tissue growth and repair, immune function, and energy production.

Like other body tissues, muscle proteinsTrusted Source are continuously broken down and rebuilt. In order to build muscle, a person must consume more protein than what is broken down. This is often referred to as a net positive nitrogen balance, as protein is high in nitrogen.

If a person is not consuming adequate amounts of protein, their body tends to break down muscleTrusted Source to provide the body with the amino acids needed to support body functions and preserve more important tissues. Over time, this can lead to decreased muscle mass and strength.

Lastly, the body uses amino acids for muscle protein synthesis (MPS), the primary driver of muscle repair, recovery, and growth after strenuous exercises.

According to the 2020-2025 Dietary Guidelines for AmericansTrusted Source, most healthy adults over 19 years old should get between 10-35% of their daily calories from protein. One gram of protein provides 4 calories.

This means that a person who eats 2,000 calories per day would need to consume between 50 and 175 grams of protein per day.

The current RDA of 0.8 g per kg of body weight for protein is based on the amount required to maintain nitrogen balance and prevent muscle loss. However, extending these recommendations to active individuals who are looking to build muscle may not be appropriate.

When it comes to building muscle mass, the ideal amount of daily protein a person should consume varies depending on several factors, including age, gender, activity level, health, and other variables.

However, several studies have given us a good idea of how to calculate the amount of protein adults need for muscle gain based on body weight.

While most studies agree that higher protein intakes are associated with improvements in lean body mass and strength when combined with resistance training, the optimal amount of protein required to build muscle remains controversial.

Here is what the latest research says.

One 2020 meta-analysis published in the journal Nutrition Reviews found that protein intakes ranging from 0.5 to 3.5 g per kg of body weight can support increases in lean body mass. In particular, researchers noted that gradually increasing protein take, even by as little as 0.1 grams per kilogram of body weight per day, can help maintain or increase muscle mass.

The rate of increase in lean body mass from higher protein intakes rapidly decreased after 1.3 g per kg of body weight was exceeded. Strength training suppressed this decline. This suggests that increased protein intake paired with strength training is best for gaining lean body mass.

Another 2022 meta-analysis published in the journal Sports Medicine concluded that higher protein intakes of around 1.5 g per kg of body weight daily paired with resistance training are required for optimal effects on muscle strength. Researchers noted that the benefits of increased protein intake on strength and muscle mass appear to plateau at 1.5 to 1.6 g per kg of body weight per day.

Lastly, one 2022 systematic review and meta-analysis published in the Journal of Cachexia, Sarcopenia, and MuscleTrusted Source concluded that a protein intake of 1.6 g per kg of body weight per day or higher results in small increases in lean body mass in young, resistance-trained individuals. The results on older individuals were marginal.

Notably, 80% of studies examined in this review reported participants consuming a minimum of 1.2 g of protein per kg of body weight per day, which is still higher than the current RDA. This may be a potential contributor to the decreased effects of protein intervention in combination with resistance training in older adults.

While it is difficult to give exact figures due to varying study results, the optimum amount of protein for muscle-building appears to be between 1.2 and 1.6 g per kg of body weight.

This means a 180-pound (81.8 kg) male, for example, would need to consume between 98 and 131 g of protein daily, combined with resistance training, to support muscle growth.

A person can meet their daily protein needs by eating animal and plant-based protein sources.

Animal-based protein sources include:

  • lean meats (beef, pork, or lamb)
  • poultry
  • eggs
  • fish and seafood
  • dairy products
  • whey protein powders.

Plant-based protein sources include:

  • beans
  • peas
  • nuts
  • lentils
  • seeds
  • soy products
  • plant-based protein powders.

Some nutritionists consider animal protein sourcesTrusted Source to be better than plant-based protein sources when it comes to building muscle mass. This is because they contain all the essential amino acids the body needs in sufficient amounts. They are also easy to digest.

Some plant-based proteins are less bioavailable and harder to digest. They also have varying amino acid profiles. However, individuals who opt for plant-based diets can easily supplement by eating more overall protein, and opting for a variety of foods.

To obtain all the necessary amino acids in a plant-based diet, individuals can pair ingredients such as rice and beans, hummus and pita bread, or peanut butter on whole wheat bread.

One notable exception is soy, which is highly bioavailable, has a good profile of amino acids, and is easy to digest.

Doctors generally agree that healthy adults can safely tolerate a long-term protein intake of up to 2 g per kg of body weight per day without any side effects. However, some groups of people, such as healthy, well-trained athletes, may tolerate up to 3.5 g per kg of body weight.

Most research suggests that eating more than 2 g of protein per kg of body weight per day can cause health issues over time.

 Semaglutide drugs, used to treat type 2 diabetes, also help with weight management. One such drug, sold under the brand name Wegovy, is approved as a treatment for overweight and obesity. But why do some people hit a weight loss plateau on Wegovy? We explore this topic with our expert guest In Conversation.

Semaglutide, or GLP-1 receptor agonist drugs, are typically prescribed to people with type 2 diabetes to help them with blood sugar management by prompting their bodies to produce more insulinTrusted Source.

These drugs can also help with weight loss by suppressing a person’s appetite. At the moment, however, only one semaglutide drug is approved as a weight management aid in people with overweight and obesity in the United StatesTrusted Source and the United Kingdom. That drug is Wegovy.

Can fiber help you lose weight? Dietitian answers 5 key questions

 When it comes to weight loss, many people track their macros — i.e., their proteins, fats, and carbohydrates. However, as a humble nutrient, fiber is often overlooked. But what if this is the missing ingredient to weight loss success? Can fiber supplements replace whole foods? In this podcast, a nutritionist answers readers’ questions about fiber and more.

Nowadays, the internet is awash with articles, charts, and recipes centered around eating more protein — anything from a 30-gram-protein breakfast to high-protein drinks and more — to naturally lose weight. And although the key to achieving good weight loss results is indeed a higher protein intake, there is a nutrient that is often overlooked: fiber.

Fiber is crucial not only for digestive functioning but overall health. Studies have shown it can lower LDL cholesterol, reduce blood pressure, and protect against heart disease. Newer research also shows that fiber may promote weight loss and enhance sensitivity to insulin.

However, statistics show that less than 5% of Americans realistically meet their recommended daily fiber intake, which is on average up to 34 grams (g)Trusted Source for adult men and about 28 g for adult women. So, how can we eat more fiber?

In this episode of In Conversation, we’ll be tackling burning questions such as: What is fiber, and why is it important for our bodies? How can we tell whether we are eating enough fiber? Is it right to call fiber nature’s Ozempic?

We’ll differentiate between soluble and insoluble fiber while discussing the ideal daily intake for different people. We’ll also touch on how fiber supplements like psyllium husk compare with whole foods, weighing their benefits for our well-being. We will also look at how fiber plays a crucial role in fighting insulin resistance and its potential role in supporting weight management goals.

To discuss this and more, we’re joined by registered dietitian Lisa Valente, MS, RD. Lisa holds a Master of Science in nutrition communications from the Friedman School of Nutrition Science and Policy at Tufts University, and she completed her dietetic internship at Massachusetts General Hospital.

After the podcast recording, for the readers of MNT, we also asked Lisa what her top favorite high fiber foods were.

“I will say frozen berries, fresh too, but frozen berries tend to be a little bit higher in fiber for a fruit. And when you buy them frozen, they’re just more affordable and easier to have on hand. [T]hey don’t go bad on you in a day — so, you can add those to smoothies or mix them into oatmeal or yogurt,” she said.

Lisa said her second choice would be whole wheat pasta and shared a fun fact about fiber.

“Brown rice only has one gram more of fiber than white rice, but whole wheat pasta has significantly more than white pasta. [It creates] like this nice fiber-rich carbohydrate base to your dinner to add some vegetables or protein to,” she said.

Her third choice was chia seeds.

“I sprinkle them on oatmeal, but you can also make chia seed pudding where you soak them with some milk or non-dairy milk and add a little bit of fruit. They’re very filling and they not only have fiber, but they also have omega-3 and a little bit of protein. So I feel like they’re sort of this tiny but mighty little seed where you can check off a lot of nutrition boxes at once. So if I had to pick three, that’s a good starting place for me,” she told us.

Temporary anxiety impacts learning

 A brief episode of anxiety may have a bigger influence on a person's ability to learn what is safe and what is not. The research recently published in NPJ Science of Learning used a virtual reality game that involved picking flowers with bees in some of the blossoms that would sting the participant -- simulated by a mild electrical stimulation on the hand.

Researchers worked with 70 neurotypical participants between the ages of 20 and 30. Claire Marino, a research assistant in the ZVR Lab, and Pavel Rjabtsenkov, a Neuroscience graduate student at the University of Rochester School of Medicine and Dentistry, were co-first authors of the study that found that the people who learned to distinguish between the safe and dangerous areas -- where the bees were and were not -- showed better spatial memory and had lower anxiety, while participants who did not learn the different areas had higher anxiety and heightened fear even in safe areas.Surprisingly, they discovered that temporary feelings of anxiety had the biggest impact on learning and not a person's general tendency to feel anxious.

"These results help explain why some people struggle with anxiety-related disorders, such as PTSD, where they may have difficulty distinguishing safe situations from dangerous ones," said the senior author of this study, Benjamin Suarez-Jimenez, PhD, associate professor of Neuroscience and Center for Visual Science at the Del Monte Institute of Neuroscience at the University of Rochester.

"The findings suggest that excessive anxiety disrupts spatial learning and threat recognition, which could contribute to chronic fear responses. Understanding these mechanisms may help improve treatments for anxiety and stress-related disorders by targeting how people process environmental threats."

Suarez-Jimenez explains that it is now important to understand if individuals with psychopathologies of anxiety and stress have similar variations in spatial memory.

Adding an attention-tracking measure, like eye-tracking, to future studies could help determine whether a focus on potential threats impacts broader environmental awareness.

Children's reading and writing develop better when they are trained in handwriting

 Today, it is common for children's classrooms to have digital resources to be used as tools for certain learning processes. For example, there are computer programs geared towards children who are learning to read and write. Since the exercises that they propose are to be done on computer, the students press keys and buttons, and do away with pencil and paper. To measure the impact of these typing-based methods, a UPV/EHU study made a comparison to analyse the effects of manual and keyboard training on children's skills. "As children write less and less by hand, we wanted to explore the impact of this on alphabetic and orthographic skills. In other words, we wanted to see whether the ability to learn letters and to assimilate and remember word structure develops differently through manual training or the use of keyboards. We concluded that the children who used their hands obtained the best results," explained researcher Joana Acha.

To reach this conclusion, an experiment was conducted with 5 to 6-year-olds. This age was chosen because it is the most favourable moment in their development. This is in fact when they begin to acquire the ability to read and write. So 50 children with basic reading comprehension were taught 9 letters of the Georgian and Armenian alphabets, as well as 16 pseudowords invented by the researchers by combining the letters."The aim was to use letters and words that were completely new to the children to make sure they were learning from scratch. In fact, the studies carried out so far used the alphabets in the children's culture, so it is not so easy to find out the extent to which they did not know the symbols presented," said Acha.

So all the students were taught new letters and words, but not all in the same way: half of them were asked to copy them by hand and the other half with keyboards. That way the UPV/EHU study was able to focus on the importance of the graphomotor function. In other words, what kind of effect does moving the hand have on the reading and writing process. In fact, when we write on keyboards we do not trace the shape of the letter and so the graphomotor function exerts less influence when it comes to assimilating letter and word structure. By contrast, writing by hand exerts a greater influence.

"Once we had taught each group of children the new letters and words and trained them using one method, we submitted them to three tests to assess the knowledge acquired. We measured their ability to identify, write and pronounce both the letters and the pseudowords, and the results clearly indicated that those who had practised manually developed greater skill. In particular, the difference was clear with the pseudowords; almost everyone who had learnt on computer did not complete the exercises on letter sequences correctly. So our work confirms that the graphomotor function is essential in memorizing letters and word structures," explained Acha.

Variability also exerts an influence

But they did not look at the impact of the degree of hand movement alone. The groups who were working by hand and with keyboards were divided into two subgroups from the start. During the teaching of the letters and the words, among those who were working with pencils, some were asked to follow the guides marked with small dots (technique of low variability). The others, by contrast, practised without any reference at all: copying freely on to the blank page (great variability). They did the same with those who were working on computer: some always used the same font for training (e.g. Tahoma), and others, more than one. That way, the researchers were able to analyse the influence of the shape variability factor in addition to the grapho-motor function.
